Emergency Requests

Records will normally be shipped from the federal records center (FRC) to NIH within 24 hours of receipt of a request. Emergency situations are those which pertain to audits, congressional inquiries, and Freedom of Information Act (FOIA) requests.

If your office has an emergency request to retrieve records from an FRC, you should complete and send/fax a copy of the Optional Form 11 (OF-11), Reference Request-Federal Records Centers, to the IC Records Liaison. The IC Records Liaison will review the request and forward it to the Washington National Records Center (WNRC). An emergency request is submitted to the WNRC by fax at 301-778-1534. The following information should be provided in the Remarks portion the OF-11:

  • Mark Urgent on the form. Specify the delivery method.
  • Identify Congressional Office. The FRC will send records directly to the congressional office requiring them. However, for any records subject to the Privacy Act of 1974, NIH offices must ensure that the specific disclosure to Congress is authorized before directing the FRC to send the records to the congressional office.
