Requesting Records
OF-11, Sections II and III
The second and third sections of Optional Form 11 (OF-11), Reference
Request-Federal Records Centers, pertain to retrieval and delivery
of the records.
Section II of the OF-11 is used by the federal records center
(FRC) when retrieving the records.
Section III consists of your contact and delivery information.

- Under Name of Requestor, Telephone No., and Date,
the requestor must match the organization identified in Item 3 on
the Standard Form 135 (SF-135), Records Transmittal and Receipt.
Only the organization or office which owns the records may issue a
request.
- Under Name and Address of Agency, the information needed
depends on the method of access. Although a records request may only
be issued by the owning organization or office, individuals outside
the organization or office may receive the records.
- To have the records delivered via USPS, supply the name and address
of the person receiving the records.
- To have the records delivered via FedEx or other expedited service,
supply the name and address of the person receiving the records.
Include a FedEx account number or other expedited service account
number in the remarks portion of Section I.
- To have the records picked up, write "PICK-UP" and indicate
in the remarks portion of Section I the name and affiliation (courier
company, NIH employee, NIH Transportation, etc.) of the person picking
up the records. This will constitute written authorization giving
permission to release the records. This individual will be required
to show photo ID upon pickup.
- Write "REVIEW" and indicate in the remarks portion of
Section I the date when the records will be reviewed at the FRC.
