Scanning Information: NCS Pearson

NCS Pearson is one source of scannable DHQ forms and scanning services. See Scanning the DHQ for information regarding other sources. Any new information we know of regarding economical printing, scanning, or data entry methods will be posted on this Web site as it becomes available.

Requirements and Costs

If you have a large scale study requiring 2000 questionnaires or more, or a study for which you have made changes to the standard DHQ-1 form, you may purchase forms from NCS Pearson.

The current price for printing a minimum order of 2000 DHQ-1 forms is $5,500. If you are making modifications to the DHQ-1, the cost of making the changes is about $100/hour. Printing costs change depending on the length of the instrument.

The DHQ-1 forms used in studies at the NCI were originally printed as machine-readable forms by NCS Pearson. To order DHQ-1 forms from NCS Pearson, contact:

The costs for scanning the DHQ-1 at NCS Pearson are:

  • $2.10 each for < 100 DHQs
  • $1.90 each for >100 DHQs
  • $50 to prepare the ASCII file needed to run Diet*Calc after scanning

Modify the Questionnaire

There is a scanning program (called ScanTools) associated with the DHQ-1 that NCI provided to NCS Pearson. If you have modified the DHQ-1, NCS Pearson will have to modify the scanning program accordingly. Any change you make to the scanning program costs a minimum of $250. Depending on the number and extent of your changes, the costs for modifying the scanning program will increase. If you add a full page to the instrument, the cost associated with modifying the scanning program is estimated to be about $500.


Sample DHQ and Diet*Calc Files for Forms Provided by NCS Pearson

NCS Pearson-style instruments, coding documentation, ScanTools program, and a Diet*Calc data dictionary are available for each version of the DHQ. In addition to these files, you will need the most recent nutrient database. The questionnaire files (PDF) are provided as samples and can not be scanned.

The DesignExpert files are provided for the DHQ-1 2007 version of the questionnaire. The art files are provided separately from the questionnaire because the pictures are not actually in the file, they are just linked. The links will need to be restored in order for the pictures to appear.
