Requesting Assistive Technology

If you are a USDA employee with a qualified disability, assistive technology accommodations may be provided for you at no cost to your agency. If you are unsure of your technology needs, please contact the TARGET Center to help determine your appropriate accommodation.

If you know the technology that you need, and would like to request Assistive Technology through the USDA TARGET Center, follow these steps:

  1. Open the USDA/CAP Partnership Request Form.
  2. Contact your DEPM to let them know you are making a request.
  3. Obtain the proper signatures and fax the form to 202-720-2681. You may also drop your form off in person at the TARGET Center.

*In addition to the above form, those requesting accommodations through the USDA/CAP partnership must also contact their agency’s Disability Employment Program Manager and provide them with Appendix A of the Reasonable Accommodations Booklet.

Examples of AT that the USDA/CAP Partnership will cover:
Screen readers, screen magnifiers, speech recognition software, and more…

Examples of AT that the USDA/CAP Partnership does NOT cover:
Large computer monitors, scooters, most ergonomic furniture, and more…

PDF files can be viewed in Adobe Acrobat Reader.
