Family History Validation Questionnaires
The Family History Validation Study used three questionnaires:
a telephone screener to select respondents, a two-part
respondent Family History of Cancer Questionnaire (FHCQ), which was administered in two
consecutive interviews over a 30-day period, and a Relative Questionnaire administered to
randomly sampled relatives or their next of kin.
The Telephone Screener:
- Identified a household member between the ages of 25 and 64 years.
- Determined eligibility, which included being raised by at least one biological
relative and having both parents, or at least one parent and sibling, born or raised in
the United States or Puerto Rico.
The Respondent FHCQ collected the following information:
Part 1:
- Respondent's identifying information, cancer history, and if cancer was present, type, year
or age of diagnosis, state of residency and place of diagnosis.
- Roster of all first- and second-degree relatives; for each relative, vital status,
year of birth or age, cancer history, and if cancer was present, type, and year or age of diagnosis.
Part 2:
- Identifying information for the relatives that were randomly sampled for cancer
validation, and if cancer was present, state of residency and place where
diagnosed.
- Permission to contact sampled relative or next of kin for an interview.
- Respondent's assessment of family cohesion with each sampled relative, and
the immediate and extended family.
The Relative Questionnaire collected the following information:
- Relative's identifying information
- Relative's cancer history, and if cancer was present, type, year or age of diagnosis,
state of residency and place of diagnosis.
