Diet History Questionnaire 2007 Paper-based Forms
The files for the DHQ paper-based forms were updated in February 2007 with changes to the Today's Date field to include the years 2007-2011.
Sample questionnaire forms, coding instructions, and the questionnaire data dictionary associated with each scanning method and for keypunch have been provided. New scanner programs for NCS Pearson and Cardiff's TELEform have also been provided.
If you are beginning a new study and plan to use paper-based forms to capture the responses, download the DHQ1.2007 files. If you have already begun a study using the previous version of the DHQ that had the years in the Today's Date box go up to 2006, only the Optimum Solutions Corp and Data Entry methods can accommodate using those forms in 2007.
