If you are having problems with a federal agency, Ginny will do her best to help you find solutions and get you the help you deserve.
Before Ginny can make inquiries on your behalf to federal agencies, however, she needs to have a signed document authorizing her office to request your personal information. By law, all Members of Congress are required to have a completed privacy authorization from (PAF), signed by you and on file in their office.
Because there are many different types of requests that Ginny is asked to help with, please find the form below that best meets your specific needs.
