Thank you for contacting Senator Martinez regarding a milestone event. Please carefully review the guidelines below prior to submitting your request.


Guidelines:

1. FLORIDA RESIDENTS: We ask that the recipient be a legal resident of Florida.

2. REQUIRED INFORMATION: Please provide the following information when making your request:

  • Salutation and Name of honoree(s).
  • Address of honoree(s) or special mailing instructions/request.
  • Date of event (month, day, year).
  • Requestor's name and daytime phone number.

Include the following information for one of the types of congratulatory letters:


  • EAGLE SCOUT OR GIRL SCOUT GOLD AWARD
    Please include the troop number of the honoree(s).
  • RETIREMENT (MILITARY OR PUBLIC SERVICE)
    Please provide the rank of honoree(s), agency, retirement date and the length of service.
  • GRADUATION
    Please include the name of the high school, college or graduate school and the type of degree(s) earned.
  • BIRTH OF BABY
    Letter requests will be fulfilled within the first year of the baby's birth. Please include full name of baby (or babies) and parents, as well as the date of birth.

GROUP REQUESTS: Organization requests for multiple greetings (more than 10) must be submitted individually with the required information listed above.

PROBLEMS USING THE WEB SITE: If you experience problems making an online request, please fax or mail your request to the Orlando, Florida Office.

ARRIVAL OF GREETING: A letter will be mailed approximately three weeks after the request is received. For celebrations of births, a letter will be mailed after the event date.
