National Violent Death Reporting System Implementation Manual Appendix I

Death Certificate

Depicted here is the U.S. Standard Certificate of Death to be introduced in some states in 2003. It contains fields to be filled out by both the Funeral Director and the Medical Certifier. It includes fields for the decedent’s personal information, demographics, location of injury, and cause of death.
