Enacted by the Deficit Reduction Act of 2005, the Money Follows the Person (MFP) Rebalancing Demonstration is part of a comprehensive, coordinated strategy to assist States, in collaboration with stakeholders, to make widespread changes to their long-term care support systems. With the history and strength of the Real Choice Systems Change (RCSC) grants as a foundation, this initiative will assist States in their efforts to reduce their reliance on institutional care, while developing community-based long-term care opportunities, enabling the elderly and people with disabilities to fully participate in their communities.
In 2007, 30 States and the District of Columbia were awarded grants. Information on MFP maintenance of effort and a summary of the awards are available for download below. The Centers for Medicare & Medicaid Services (CMS) awarded $1,435,709,479 in MFP grants with States proposing to transition 37,731 individuals out of institutional settings over the five-year demonstration period.
Department of Housing and Urban Development Letter in Support of MFP
Secretary for the Department of Housing and Urban Development (HUD) Alphonso Jackson informed Public Housing Authorities and State Housing Finance Agencies that the MFP initiative provides an opportunity for them to collaborate with the CMS and disability organizations to expand accessible, affordable, and integrated housing options for persons with disabilities and seniors. Further, he encouraged them to use mechanisms at their disposal to advance the expansion of this housing. The Secretary's letter can be downloaded below. MFP Reports from the Field Number 1: Transitioning Medicaid Enrollees from Institutions to the Community: Number of People Eligible and Number of Transitions Targeted Under MFP This is one of an ongoing series of reports being completed as part of the CMS MFP National evaluation. This first report assesses the scope of the program—profiling the Medicaid population in long-term institutional care eligible for MFP, their rates of transition to the community before MFP began, and the potential change in transition rates if States reach their program goals. State MFP programs seek to transition about 36,000 individuals over the course of the demonstration.
