Chronic Diseases: The Leading Causes of Death
Profiles the leading causes of death in the United States; such as, heart disease, stroke, cancer, and diabetes. Chronic diseases are among the most prevalent, costly, and preventable of all health problems. About 7 of 10 Americans who die each year, or more than 1.7 million people, die of a chronic disease.

State Cancer Facts
The State Cancer Facts application provides state-specific data on lung cancer, colorectal cancer, breast cancer, and prostate cancer, including the estimated number of new cancer cases, cancer deaths, and the age-adjusted mortality rates for cancer deaths by race for each state.

Legislation

Nutrition and Physical Activity Legislative Information
This state-based searchable database provides legislation information pertaining to nutrition, physical activity and obesity. It provides bill numbers, year, congressional sponsor; a short summary of the legislation; and its current status.

State Legislative Information: Search for Bills
Topics related to the main subjects of nutrition, physical activity and the combination of the two are featured in this database. Topics such as obesity/overweight are included, as well as more specific features such as location ( i.e.: schools, worksite), related focus area (i.e.: Fruits and Vegetables), or service (i.e.: Health Insurance). Topic areas not covered are breastfeeding and areas related to medical nutrition, such as meal replacements in hospitals.

Reports

School Health Program Report Card
Information from the School Health Policies and Programs Study (SHPPS) 2006 evaluating essential school health program components. The Report Card addresses performance in health education, physical education, health services, mental health and social services, food service, school policy and environment, faculty and staff health promotion, and family and community involvement.

The State of Aging and Health in America
The State of Aging and Health in America report assesses the health status and health behaviors of U.S. adults aged 65 years and older and makes recommendations to improve the mental and physical health of all Americans in their later years. The report includes national- and state-based report cards that examine 15 key indicators of older adult health, Calls to Action which recommend strategies to improve the health and quality of life of older adults, and state examples that highlight innovative healthy aging efforts at the state and community level.

Tobacco Control Highlights Report
This report highlights current state-based information on the prevalence of tobacco use, the health impact and costs associated with tobacco use, and tobacco excise tax drawing from multiple sources within the State Tobacco Activities Tracking and Evaluation (STATE) System. The STATE System is an electronic data warehouse containing up-to-date and historical state-level data on tobacco use prevention and control.

State Based Programs/Projects

Adolescent and School Health Programs
CDC's Division of Adolescent and School Health (DASH) supports state, territorial, and local agencies and tribal governments to help build and strengthen their capacity to improve child and adolescent health.

Dental Public Health Programs: Synopses by State
Provides contact information, a demographic profile, and summary statistics on key oral health indicators: the state's public water system and fluoridation infrastructure; its public health workforce and administration of dentistry; and its specialized oral health programs. Allows the tracking of trends for selected variables.

Diabetes Control Program
A summary of activities supported by the State-Based Diabetes Control Program. Provides information on how CDC funding supports activities such as surveillance, community-based program management, collaborative research, and heath-care management.

Healthy Youth
CDC's Division of Adolescent and School Health (DASH) supports state, territorial, and local education agencies to help build and strengthen their capacity to improve child and adolescent health.

National Breast and Cervical Cancer Early Detection Program
Shows the range of CDC's capacity-building programs that assist states in the early detection of breast and cervical cancers. Locate state program contact information, access scientific data and legislative records, and find clinics where free or low-cost mammogram and Pap tests are offered.

National Comprehensive Cancer Control Program
Program contact information by state. The National Comprehensive Cancer Control Program serves as a focal point for exchanging information about comprehensive cancer control in general as well as specific information about state approaches.

National Program of Cancer Registries Program
Provides state program contact information as well as reference information about funded National Cancer Registries Programs: registry links, state program profiles, and cancer burden data.

Physical Activity Program Directory
Information on a variety of state programs addressing physical activity and health. Includes profiles of partner organizations; project information such as status, scope, target population, setting, and purpose; program components, evaluation, and products; and individual program contacts.

2005 Assisted Reproductive Technology Success Rates Report
The 2005 report of pregnancy success rates is the ninth report to be issued under the Fertility Clinic Success Rate and Certification Act. The report includes a national overview that uses information from 411 U.S. fertility clinics to provide an in-depth picture of the type, number, and outcome of ART cycles performed in U.S. clinics. The report also includes individual clinic tables that provide ART success rates and other information from each clinic that submitted and verified its 2005 data and appendixes containing the results of data validation visits, technical notes, a glossary, and contact information for reporting and nonreporting clinics in the United States.

Behavioral Risk Factor Surveillance System Prevalence Data
The Behavioral Risk Factor Surveillance System (BRFSS) allows users to query prevalence data on up to 24 behavioral risk factors, for a reporting area that currently includes all 50 states, the District of Columbia, and three territories.

Diabetes Data & Trends
The Data and Trends section, which includes the National Diabetes Fact Sheet and the National Diabetes Surveillance System, provides resources documenting the public health burden of diabetes and its complications in the United States. The section also includes state surveillance data including county maps to monitor state-specific trends over time of diabetes.

Fruit and Vegetable Consumption
Compare fruit and vegetable consumption by age, body mass index (BMI), education, gender, income, physical activity, and race. Further compare fruit and vegetable consumption nationwide to your state, compare your state to another state, or view trends over time.

Heart Disease and Stroke Maps
These interactive state maps present heart disease and stroke mortality rates, by racial/ethnic group and by gender. Summary data is presented for each state in a fact sheet and profile table. Detailed data by county is also available.

Most Common Cancers
Includes bar graphs ranking the age-adjusted invasive cancer incidence rates for the 10 primary sites with the highest rates within sex, race- and ethnic-specific populations in each state or metropolitan area.

Oral Health State Profiles
Key oral health statistics on dental visits, teeth cleanings, complete tooth loss, and community water fluoridation status. Also provides state dental program contact information, infrastructure and population profiles, and links to related information. Data are drawn from the CDC National Oral Health Surveillance System (NOHSS).

State-based Physical Activity Statistics
Physical activity data are pulled from the Behavioral Risk Factor Surveillance System (BRFSS), a national telephone survey conducted by the Centers for Disease Control and Prevention (CDC) and state health departments. The BRFSS physical activity questions attempt to measure a person’s physical activity in leisure-time, household, and transportation.

State Tobacco Activities Tracking and Evaluation (STATE) System
The STATE System is an electronic data warehouse containing up-to-date and historical state-level data on tobacco use prevention and control. Main topic areas presently being offered are behaviors, demographics, economics, funding, health consequences and costs, and legislation.

YRBSS Youth Online: Comprehensive Results
Provides interactive access to information from the CDC Youth Risk Behavior Surveillance System (YRBSS). View reports and graphs summarizing survey research on typical risk factors among youth: unintentional injuries/violence, tobacco use, alcohol/other drug use, sexual behaviors, dietary behaviors, physical activity, and others.
