Un apprentissage qui dure toute la vie
Ce mois-ci, la rubrique NIEHS News (p. A806) passe en revue les moyens d'éducation relatifs à l'hygiène du milieu proposés par l'institut NIEHS à des étudiants de tout âge ainsi qu'à des enseignants et des scientifiques dans l'ensemble des États-Unis. D'autre part, des personnes concernées se sont réunies pour discuter des recommandations cliniques et les objectifs de la recherche relatifs à l'exposition à la moisissure toxique, et l'étude sœur parrainée par le NIEHS a été lancée à l'échelle nationale pour étudier comment la réaction entre les facteurs génétiques et environnementaux peut entraîner le cancer du sein.
L'hygiène du milieu sort en librairie
Des initiatives sont en cours partout aux États-Unis pour introduire l'enseignement de l'hygiène du milieu dans les programmes scolaires afin d'encourager les étudiants à devenir des citoyens bien informés et capables d'évaluer les questions importantes concernant la santé publique et d'agir en conséquence. Certains éléments de notre système scolaire entravent l'incorporation de l'hygiène du milieu dans le programme d'études. La rubrique Focus (p. A814) examine la présence de l'enseignement de l'hygiène du milieu dans le système scolaire actuel aux États-Unis et décrit les efforts déployés par plusieurs groupes pour sensibiliser les étudiants à leurs environnements.
Exploitations agricoles, sources d'engraissement ?
Malgré les tentatives pour réduire la production agricole, les programmes de soutien agricole ont plus d'influence que jamais et encouragent les agriculteurs à cultiver des quantités toujours plus importantes de blé, de soja et notamment de maïs. C'est également la taille des Américains qui s'élargit plus que jamais, grâce, en partie, à la forte consommation de denrées alimentaires bon marché produites à partir de ces cultures. La rubrique Sphères d'influence (p. A820) examine les programmes de soutien et de subvention agricole et la mesure dans laquelle le fait de soutenir les agriculteurs à produire certaines cultures contribue à stimuler l'épidémie de l'obésité américaine.

Méthode de référence modifiée pour le développement des recommandations relatives au BaP
Des évaluations des risques de cancer provoqué par l'exposition aux hydrocarbures aromatiques polycycliques (PAH) ont été réalisées par l'application du modèle à plusieurs étapes, linéarisé et conservateur à des données de tumeurs chez les animaux pour le benzo(a)pyrène (BaP). Fitzgerald et al. (p. 1341) se sont servis d'une série de modèles mathématiques classiques appliqués aux données d'effet dose relatives aux tumeurs pour mettre au point une méthode de dose de référence modifiée. Des données relatives aux tumeurs découlant de l'ingestion de mélanges de goudron de houille, chez les souris, remettent en question l'utilisation habituelle des facteurs d'équivalence de toxicité basés sur le BaP pour l'évaluation des risques des mélanges complexes de PAH.
Perturbation mitochondriale par des produits chimiques DEP aromatiques et polaires
Les polluants particulaires provoquent des effets indésirables sur la santé en générant un stress oxydatif. Une question essentielle est de savoir si ces effets sont médiés par les particules elles-mêmes ou par leurs composés chimiques. Xia et al. (p. 1347) montrent que les composés organiques aliphatiques, aromatiques et polaires, fractionnés à partir de particules toxiques d'échappement (DEP - diesel exhaust particles) émises par les véhicules diesel, entraînent des effets toxiques différentiels dans des cellules. L'utilisation de DEP intacts ainsi que de particules ultrafines ambiantes a permis de reproduire les effets chimiques sur des mitochondries isolées. Par opposition, les nanoparticules polystyrènes commerciales n'ont pas produit d'effet mitochondrial. Ces résultats semblent indiquer que les effets des DEP et des particules ultrafines ambiantes sur le potentiel des membranes et des pores de transition de perméabilité des mitochondries sont médiés par des produits chimiques adsorbés au lieu des particules elles-mêmes. Infections et expositions aux organochlorés chez des nourrissons
La population inuit de Nunavik (Canada) est exposée à des organochlorés (OC) immunotoxiques par la consommation de poisson et de graisse de mammifères marins. Dallaire et al. (p.1359) ont fait des recherches sur l'effet de l'exposition périnatale aux polychlorobiphényles (PCB) et au DDE sur l'incidence d'infections aiguës chez des nourrissons inuits. En comparaison aux taux des nourrissons se trouvant dans le premier quartile d'exposition aux PCB (les moins exposés), les rapports de taux ajustés des nourrissons se trouvant dans des quartiles supérieurs se situaient entre 1,17 et 1,27 pour toutes les infections confondues pendant les premiers six mois du suivi. Les résultats attestent une association éventuelle entre l'exposition prénatale aux organochlorés (OC) et les infections aiguës au début de la vie chez cette population.
La santé infantile
Pollution particulaire de l'air et mortalité infantile
Des augmentations de la pollution de l'air ambiant à court et à long terme sont en rapport avec la mortalité et la morbidité accrues chez des adultes et des enfants. Glinianaia et al. (p. 1365) réexaminent les arguments épidémiologiques en faveur d'une association entre la pollution particulaire de l'air et la mortalité infantile. Les arguments, qui étaient rapportés concernant des endroits dont les taux de pollution étaient en grande partie comparables, étaient généralement contradictoires. Certains arguments démontrent que le poids de l'association avec la matière particulaire différait par sous-groupes de mortalité infantile.
Concentrations en nitrates dans l'eau potable et la méthémoglobinémie
On s'est accordé précédemment à reconnaître que la consommation d'eau potable avec une concentration élevée en nitrates provoque la méthémoglobinémie chez des nourrissons. Fewtrell (p. 1371) expose les grandes lignes d'une étude sur la concentration en nitrates dans l'eau potable et la méthémoglobinémie qui découle des concentrations en nitrates dans cette eau supérieures au taux prescrit par l'Organisation mondiale de la santé de 50 mg/l. Étant donné l'incidence apparemment faible d'une éventuelle méthémoglobinémie provoquée par l'eau et la complexité du rôle des nitrates et du comportement individuel, il est actuellement inopportun de chercher à établir un rapport entre les taux de maladie et les concentrations en nitrates dans l'eau potable.
Arsenic sur les mains des enfants
L'exposition des enfants à l'arsenic lorsqu'ils jouent dans des cours de récréation construits avec du bois traité à l'arséniate de cuivre chromé (CCA) fait l'objet de préoccupation grandissante. Kwon et al. (p. 1375) ont déterminé que la quantité moyenne d'arsenic sur les mains des enfants ayant joué dans des cours de récréation traités au CCA était considérablement plus élevée que celle des enfants ayant joué dans des cours de récréation sans CCA. Il n'existait pas de différence appréciable dans la quantité de sable se trouvant sur les mains des enfants et la concentration d'arsenic dans le sable entre les groupes CCA et sans CCA. La quantité d'arsenic maximale trouvée sur les mains de l'ensemble des enfants participant dans le groupe étudié était inférieure à l'apport quotidien moyen d'arsenic provenant des denrées alimentaires et de l'eau. Effet des concentrations de plomb dans le lait maternel sur les concentrations sanguines en plomb chez les nourrissons
Ettinger et al. (p. 1381) ont étudié le rapport entre les concentrations de plomb dans le lait maternel et les concentrations sanguines en plomb chez les nourrissons chez des couples mère-nourrisson. Les concentrations de plomb dans le cordon ombilical et dans le sang maternel étaient mesurées au moment de l'accouchement, ainsi que dans le sang, les os et le lait maternel et dans le sang du nourrisson un mois après l'accouchement. La concentration sanguine en plomb chez les nourrissons à un mois postpartum était nettement en corrélation avec les concentrations de plomb dans le cordon ombilical et dans le sang maternel au moment de l'accouchement, et avec les concentrations de plomb dans le sang, la rotule et le lait maternels à un mois postpartum. Bien que les concentrations de plomb dans le lait maternel furent faibles, leur influence sur les concentrations sanguines en plomb chez les nourrissons était plus importante que celle de la concentration de plomb dans le sang maternel.
Expositions aux COV chez des enfants
Adgate et al. (p. 1386) ont mesuré des expositions aux composés organiques volatiles (COV) à plusieurs endroits de la ville chez une population diverse d'enfants qui allaient à deux écoles dans des quartiers déshérités du Minneapolis, Minnesota. Ils ont mesuré 15 COV courants à quatre endroits: à l'extérieur (E), à l'intérieur à l'école (IE), à l'intérieur à la maison (IM) et des échantillons personnels (P). Les concentrations de la plupart des COV correspondaient au modèle général suivant : E IM < ( P IM des microenvironnements mesurés. Les expositions IM et P médianes et supérieures étaient nettement au-dessus des seuils de tolérance pour plusieurs composés. Les mesures extérieures pourraient ainsi conduire à une sous-estimation des risques pour la santé à long terme de l'exposition infantile à ces composés.
Exposition au phtalate à l'intérieur et symptômes allergiques
La production mondiale de phtalate ester a augmenté pour atteindre environ 3,5 millions tonnes/année. Bornehag et al. (p. 1393) ont examiné les associations entre les symptômes allergiques persistants chez des enfants et les concentrations de phtalates dans la poussière recueillie de leurs maisons à l'aide d'une étude cas-témoins auprès d'une grande cohorte d'enfants. Les résultats signalent que les phtalates, dont la concentration se situaient dans les limites de ce qui est généralement observé dans des environnements intérieurs, sont associés à des symptômes allergiques chez certains enfants. Les différentes associations de symptômes des trois phtalates principaux peuvent s'expliquer par une combinaison des propriétés physiques et du potentiel toxicologique des produits chimiques.
Croissance du fœtus et exposition à des particules fines pendant la grossesse
Jedrychowski et al. (p. 1398) ont estimé les effets de l'exposition à la matière particulaire fine (PM2.5) chez des femmes enceintes en Pologne sur les incidences congénitales. La cohorte se composait de femmes enceintes qui ont accouché entre 34 et 43 semaines de gestation. Les mesures de PM2.5 étaient réalisées à l'aide d'un contrôle pneumatique de chaque femme durant 48 heurs pendant le deuxième trimestre de la grossesse. La réduction théorique du poids à la naissance avec une augmentation d'exposition de 10 à 50 mg/m3 était de 140,3 g. La réduction théorique correspondante de la taille à la naissance et du périmètre crânien serait respectivement de 1,0 cm et de 0.5 cm. L'étude a mis en avant de nouveaux arguments épidémiologiques confortant l'association entre une forte exposition personnelle à des particules fines et des effets défavorables subis par le fœtus in utero.
TCDD et incidences congénitales
Lawson et al. (p. 1403) ont étudié les dénouements fâcheux de la grossesse parmi les femmes de travailleurs fortement exposés à des produits chimiques contaminés à la TCDD et parmi les femmes d'hommes non exposés vivant dans les environs. Ni la concentration en TCDD continue, ni la concentration en TCDD formelle n'avaient d'effet sur le poids à la naissance chez les enfants nés à terme après l'ajustement du sexe du nourrisson, du niveau d'études de la mère, du nombre d'enfants nés de la même mère, du tabagisme prénatal et de la durée de la gestation. Étant donnée que les concentrations en TCDD estimées chez cette population étaient beaucoup plus élevées par rapport à d'autres études, les résultats indiquent qu'il est peu probable que la TCDD augmente le risque d'un faible poids à la naissance ou d'un accouchement prématuré par l'intermédiaire d'un mécanisme paternel.
Contrôle de la santé publique liée à l'environnement au niveau national
Les centres américains de prévention et de contrôle de la maladie (U.S. Centers for Disease Control and Prevention) ont lancé le programme de contrôle de la santé publique liée à l'environnement (Environmental Public Health Tracking - EPHT) pour intégrer le contrôle des dangers, de l'exposition à des polluants et des effets sur la santé dans un réseau de contrôle cohésif. L'efficacité d'un tel réseau repose entre autres sur la prise en considération des questions suivantes : des standards uniformes et acceptables pour les données, des définitions de cas facilement compréhensibles et une meilleure communication entre les organisations de la santé et de l'environnement. La courte monographie sur le contrôle de la santé publique (p. 1409, 1414, 1419, 1424, 1428, 1434 et 1440) présente des stratégies et des méthodes innovatrices, utilisées dans le cadre de la mise au point de programmes EPHT pour résoudre un large éventail de problèmes importants et complexes liés à l'hygiène du milieu.
