SUBJECT: STAFF REQUIREMENTS - SECY-05-0035 - REPORT TO CONGRESS ON ABNORMAL OCCURRENCES: FISCAL YEAR 2004

The Commission has approved the Abnormal Occurrences Report to Congress for FY 2004 and the proposed forwarding letters, subject to the following comments and changes.

In the future, the staff should write these reports in such a manner where it is clearly understandable to a non-technical audience and places the events in proper perspective.

The staff should work with the Agreement States to improve the consistency and clarity of AO event descriptions for this report and future editions of this report. In particular, somewhere under the heading titled “Nature and Probable Consequences” for medical events, the report should provide two standard statements. One statement should be that the patient and referring physician were informed of the circumstances as is required by our regulations. The other statement should indicate the medical significance of the event to the patient. All of the medical events should address both issues. The description of the medical significance to the patient can be fairly short (one or two sentences). For the current report and future reports, if the event report does not provide a statement from the referring physician, or another physician, the staff should use our medical consultants to provide a statement indicating what that exposure could mean to the patient and that normally the referring physician would follow up in investigating those possibilities.

Changes to the Abnormal Occurrences Report for FY 2004

  1. On page 1, next to last paragraph, the text should clarify that the “one member” referred to in line 8 is in addition to or a subset of the “four members” referred to in line 6. The discussion in this paragraph should include a brief description of the proximate causes of the UF6 release. Delete the sentence in lines 11 through 14 (Based on air samples and ... safety.)

  2. On page 3, 2nd full paragraph, revise line 4 to read ‘ ... with prohibited Hunt Hund valves attached.’

  3. On page 3, 1st full paragraph in event 04-02, revise line 1 to read ‘ ... Criterion Criteria III.A., ....’

  4. On page 4, 1st full paragraph, revise line 1 to read ‘The licensee’s criticality ....’

  5. On page 12, event AS 04-06, verify the year as 2003.

  6. On page 15, in the paragraph “Licensee”, define “cine” in line 1.

  7. On page 16, 2nd full paragraph, line 1, spell out “IVB”.

  8. On page 28, paragraph 4, revise line 1 to read ‘ ... dispatched an Augmented Inspection Team (AIT) to PVNGS on ....’

  9. On page 29, last paragraph, revise lines 1 and 2 to read ‘ ... has been considered. Although the NRC has not formally evaluated this possibility. The information ....’

  10. On page 31, next to last paragraph, revise line 3 to read ‘ ... to an Augmented Inspection Team (AIT) investigation.’
