On February 26, 2008, a Notice of Violation was issued for a Severity
Level III violation involving deliberate misconduct on the part of
an individual which caused his employer, Baxter Healthcare of Puerto
Rico to be in violation of NRC regulations. Specifically, the individual
deliberately failed to perform required weekly, monthly, and quarterly
maintenance checks of safety systems, and deliberately created inaccurate
records of the aforementioned maintenance checks to reflect that
these activities had been completed when, in fact, they were not
completed as required by the associated regulation. |
