Help with a Federal Agency
Dealing with federal agencies can be difficult and my office can help you. One of my most important responsibilities as a Member of Congress is to assist my constituents when they encounter difficulties with federal agencies. Some of these problems may include difficulty obtaining veterans' or social security benefits, problems with the application process for US citizenship, or receiving an income tax refund. In my district offices, I have an experienced staff of case workers who are dedicated to assisting you in resolving these problems with federal agencies.
The Privacy Act of 1974 requires that Members of Congress and their staff receive written authorization before they can obtain information about an individual's case. Therefore, print and complete the form I have provided to allow my staff to properly assist you with your casework. Filling out this form will provide us with official permission to look into your case.
