I agree with whoever sent the idea...get a group of adult learners together
and brainstorm, prioritize (have them do this) and use their ideas for your
survey.  If you use the language they use it will probably work better too.

Hi, 

I need the assistance of some family literacy
colleagues. I am a "silent lurker" on the list
usually. 

I coordinate adult literacy education programs. Our
local 21st Century CLC director has asked me to create
a survey to distribute to parents asking what type of
adult ed classes they would like offered through 21st
Century. I was wondering if any of you had a similar
survey that I could use as a model? 

I want to include useful questions, but I don't want
it to be overbearing. We are thinking of a single
8x11.5 sheet. I have considered using ranking scales
(1 - low interest, 5 - high interest) or simple yes/no
choices. 

I would appreciate ideas and samples if any of you are
willing to share. They can be e-mailed to me at
