HCFA (1999) "Writing and Designing Print Materials for Beneficiaries:A 
Guide for State Medicaid Agencies." Center for Medicaid and State Operations.

CMS is also about to publish a guide on writing and designing Medicaid  and 
SCHIP Notices and Applications ( Spanish and English) - "The Notices and 
Applications Handbook: A Guide for Simplifying ( which I co-authored along 
with colleagues at MAXIMUS Inc.
