MD-Individual Practice Association Changes for 2004


This is a general description of the plan changes. This page is not an official statement of benefits. For that, go to the Benefits descriptions in the Plan Brochure. Also, we edited and clarified language throughout the brochure; any language change not shown here is a clarification that does not change benefits.

  • Your share of the non-Postal premium will increase by 5.8% for Self Only coverage or 5.4% for Self and Family coverage.

  • Prescription drugs - You now pay $20 per brand name drug on the Plan's formulary, and $35 per brand name drug not on the Plan's formulary. The copayments for generic and injectable drugs remain the same.

  • Prescription drugs - You now pay one copayment for up to a 90 day supply for contraceptive drugs purchased through a Plan retail pharmacy or the Plan's mail order program.

  • Inpatient hospitalization - You now pay a $100 copayment for each inpatient hospital admission.

  • Service area expansion - We have expanded our service area in the State of Virginia to include the City of Harrisonburg and the counties of Fluvanna, Charlotte, Rockingham, Lunenburg, and Prince Edward.
