Piedmont Community HealthCare Changes for 2004


This is a general description of the plan changes. This page is not an official statement of benefits. For that, go to the Benefits descriptions in the Plan Brochure. Also, we edited and clarified language throughout the brochure; any language change not shown here is a clarification that does not change benefits.

  • Your share of the non-Postal premium decrease by 21% for Self Only or 20% for Self and Family.

  • An in-plan deductible of $500 per person and $1000 per family was added.

  • The out-of-plan deductible amount increased to $1500 per person and $3000 per family.

  • The $20 copayment for an office visit has increased to a $25 copayment.

  • The $50 emergency room copayment increased to a $100 copayment.

  • The prescription drug copayment for generic prescriptions filled at the pharmacy, 30-day supply or 100-units, increased from a $10 copayment to a $15 copayment.

  • The prescription drug copayment for brand name prescriptions filled at the pharmacy, 30-day supply or 100-units, increased from a $20 copayment to a $30 copayment.

  • The prescription drug copayment for generic prescriptions filled through the mail order, 90-day supply or 300-units, increased from a $20 copayment to a $30 copayment

  • The prescription drug copayment for brand name prescriptions filled through the mail order, 90-day supply or 300-units, increased from a $40 copayment to a $60 copayment.

  • The in-plan out-of-pocket maximum increased from $1000 per person to $3000; and from $2000 per family to $6000 per family.

  • The out-of-plan out-of-pocket maximum increased from $2000 per person to $6000; and from $4000 per family to $12,000.
