Sharon,

One tool with which I am familiar is the Health Literacy Audit Kit that was developed by the Alberta (Canada) Association for Adult Literacy. I'm not sure that it meets your "validation" criteria, but it has some good ideas and reproducible handouts. You can inquire about the kit from the Association at this phone number or email:
Tel: 403-297-4994. email: office@aaal.ab.ca

Good luck and please let us know if you identify additional tools.

Hello! I am working on a health literacy project and was wondering if
anyone could send me suggestions or tips for locating validated health
literacy audit tools for the purpose of assessing the level of service
health care organizations (specifically, hospitals and pharmacies) offer to
low-literate adult patients. Thank you very much!
