To obtain immigration forms, please click here. Please retain for your records a copy of any form or document you submit to the USCIS. My staff assistants are unable to help complete USCIS forms. Residents of the 17th district of Illinois are under the jurisdiction of the USCIS Chicago District Office. One must make an appointment to visit that office; you may make an appointment using INFOPASS at http://infopass.uscis.gov/. The NCSC can connect you to live assistance in English and Spanish during their specified office hours.
