Integrated Care Initiative


The Integrated Care Initiative began in December 2005 when a cross-component workgroup was formed at the request of the Administrator of the Centers for Medicare & Medicaid Services (CMS).  The purpose of this workgroup was to remove administrative barriers to implementing Special Needs Plans (SNPs) and to generate State awareness of the opportunity to better integrate care for individuals who are dually eligible for both Medicare and Medicaid. The overall goal of integrated care is to provide the full array of Medicare and Medicaid benefits through a single delivery system that will provide quality of care for dual eligible beneficiaries, better care coordination and fewer administrative burdens. 

In an effort to support this initiative, the CMS workgroup met with a number of outside groups, including the National Health Policy Group (NHPG), the Center for Health Care Strategies (CHCS), and the Reforming States Group (RSG), to discuss barriers to developing integrated delivery systems. In addition, CMS worked closely with CHCS, and their State partners to identify and address challenges to integrating care for dually eligible individuals.  Based on lessons learned from the workgroup meetings with these stakeholders and State experiences from several integrated care demonstrations, CMS developed several resources that provide information and guidance related to the implementation of integrated care programs.  The "Road Map" page on this website provides a summary of, and link to, the tools and documents available to State Medicaid Agencies, health plans, and providers for use and consideration in the design and development of their integrated care service delivery model.
