STATES GET FEDERAL GRANTS TO CREATE INCREASED OPPORTUNITIES FOR INDIVIDUALS TO HAVE PERSON-CENTERED PLANS FOR HOME AND COMMUNITY-BASED SERVICES   

On September 5, 2008, the Centers for Medicare and Medicaid Services (CMS) announced over $8.3 million in grants to States to help maximize opportunities for people to live in the community and have services organized around the needs of the individuals rather than the settings where care is delivered.  Of this total grant funding, $7.7 million was awarded to States to develop "Person-centered Hospital Discharge Planning Models."  These models will be used to effectively engage individuals with disabilities and their informal caregivers with the hospital discharge planning process.  

A major objective of this grant program is to increase awareness of the options for home and community-based services so that individuals, who are at risk for institutionalization, may remain in their own homes with appropriate services.  

These grant awards are part of the Real Choice Systems Change (RCSC) Grant Program. They will build upon the successes and direction of earlier RCSC and Aging and Disability Resource Center (ADRC) Grant Program opportunities by providing targeted assistance to States in their efforts to improve hospital discharge planning through collaboration with the ADRCs, Area Agencies on Aging (AAA), and the Centers for Independent Living (CIL). 

This grant opportunity is also designed to increase the capacity of the ADRCs to provide critical linkages to hospital discharge planners, consumers and their caregivers regarding long-term care services in the community. Three States (Kansas, Alaska, and South Carolina) will use a portion of their award to enhance or expand the ADRCs in their States while two States (Oregon and Missouri) will use a portion of their award to develop new ADRCs in their States. The State of Idaho will concentrate solely on developing and implementing a person-centered hospital discharge planning model.

An additional $574,613 was awarded in supplemental grants to two States that received Person-Centered Planning Implementation Grants in FY2007.  This funding will allow these grantees to continue their successful efforts for two more years.
