Money Follows the Person Grants

Enacted by the Deficit Reduction Act of 2005, the Money Follows the Person (MFP) Rebalancing Demonstration is part of a comprehensive, coordinated strategy to assist States, in collaboration with stakeholders, to make widespread changes to their long-term care support systems. With the history and strength of the Real Choice Systems Change grants as a foundation, this initiative will assist States in their efforts to reduce their reliance on institutional care while developing community-based long-term care opportunities, enabling the elderly and people with disabilities to fully participate in their communities.

Proposals were due November 1, 2006 and 31 grants were awarded in 2007 to 31 States. The grant announcement, additional guidance related to the grant announcement, information on MFP maintenance of effort and a summary of the awards made are available for download below. CMS awarded $1,435,709,479 in MFP grants with States proposing to transition 37,731 individuals out of institutional settings over the five-year demonstration period.  Documents for use by the grantees, including the Operational Protocol and a Training Schedule are also available.

Department of Housing and Urban Development Letter in Support of MFP - Secretary Jackson informed Public Housing Authorities and State Housing Finance Agencies that the MFP initiative provides an opportunity for them to collaborate with CMS and disability organizations to expand accessible, affordable, and integrated housing options for persons with disabilities and seniors.  Further, he encouraged them to use mechanisms at their disposal to advance the expansion of this housing.  The letter can be downloaded below.
