During your visit to our Forest Service website, you might be randomly selected to take a customer-satisfaction survey on leaving the site. Your survey response will help us improve our service to you through our Forest Service website.

If you are randomly selected when leaving our site, a pop-up window will lead to the survey. The survey uses a temporary "cookie" that will appear as an extra button in the taskbar at the bottom of your screen. The cookie is erased as soon as you leave and no personal or identifying information is gathered.

If you have set up your Internet browser to ignore or disable pop-ups, you will not be able to be randomly selected for the survey.

If you have disabled JavaScript in your browser-but are interested in responding to our survey if randomly selected-you will want to enable JavaScript and reload the website now.
