LICENSE APPLICATION FOR TRAVEL TO CUBA
To visit an immediate family member in Cuba, you need to
submit an OFAC FastRequest.
Use of this license application wizard is voluntary.
Before attempting to use this wizard, you should become familiar with the licensing criteria provided in the
Comprehensive Guidelines for License Applications to Engage in Travel-related Transactions Involving Cuba
, hereafter referred to during this online process as “Application Guidelines."

must not
be viewed as a substitute for the Code of Federal Regulations.

It is the responsibility of the applicant to demonstrate and/or document that their proposed activities in Cuba fall into one of the general or specific license categories of activities listed in section 515.560(a) and related sections of the Regulations, and, with respect to qualification for specific licenses, meet the application criteria set forth in the application guidelines. OFAC may request additional information beyond that provided for in the application guidelines to determine whether an applicant qualifies for a license.
When you are finished responding to all applicable questions you will need to PRINT and SIGN the application that is generated for you, and include all required attachments before mailing your application to OFAC.
