Be sure to read and understand the Nomination Process and IQCS Nomination Process pages before proceeding.
Note: the pre-filled Nomination Form does not submit your Nomination. It is a tool to help you with the Nomination process. The pre-filled Nomination form can be used to communicate your information to your Training Officer.
- Go to the National Schedule of Course Sessions
- Find the session in which you are interested.
- In the row for that session, click on the Nom Form link.
- Fill in your information and then click the Save Form button. Your form will be digitally signed with your name when you save it.
- Once your form is complete, you can choose to create an RTF or a PDF with all your information saved within the file.
- Most folks should choose to create an RTF as an RTF is editable with a word processor such as Microsoft Word: this allows you or your Training Officer to make changes to the form. (A PDF is not editable unless you have PDF editing software.)
- Your Nomination Form can then be sent to your Training Officer via email, fax, or mail.
