| THREE SPRINKLER SYSTEMS DECLARED INOPERABLE DUE TO CORRODED HEADS (24-hour |
| report) |
| |
| The following text is a portion of a facsimile received from Paducah: |
| |
| "On 06/02/99 at 1630 CDT, the Plant Shift Superintendent (PSS) was notified |
| that numerous sprinkler heads were corroded, affecting 16 sprinkler systems |
| in C-337 and one system in C-333, such that the ability of the sprinklers to |
| flow sufficient water was called into question. Subsequently, these |
| sprinkler systems were declared inoperable, and TSR-required actions |
| establishing roving fire patrols were initiated. This deficiency was |
| detected during scheduled system inspections conducted by Fire Protection |
| personnel. Currently, functionality of the sprinkler heads has not been |
| fully evaluated by Fire Protection personnel, and the remaining cascade |
| buildings are currently being inspected, and if necessary, this report will |
| be updated to identify any additional areas. |

| system 27 in C-335 were identified to also be corroded. These were |
| identified to the PSS on 06/03/99 at 1600 CDT and 1601 CDT, respectively, |
| and determined to require an update to this report by the PSS. |
| |

| 10CFR76.120(c)(2) as an event in which equipment is disabled or fails to |
| function as designed." |

| "Two sprinkler heads on system C-15 and one sprinkler head on system B-8 in |
| C-333 were identified to have corrosion. The PSS was notified of this |
| condition at 1350 CDT on 06/18/99. The area of the fire patrol for system |
| C-15 was expanded to include the two heads identified as corroded. The one |
| head on system B-8 was in the area already being patrolled. The PSS |
| determined that an update to this report was required." |

| "Two sprinkler heads on system D-8 and three sprinkler heads on system D-7 |
| in C-337 were identified to have corrosion. The PSS was notified of the |
| condition on system D-8 at 0125 CDT on 06/25/99 and at 1019 CDT on 06/25/99 |
| for system D-7. Both systems were immediately declared inoperable and LCO |
| fire patrol actions were implemented. It was determined that an update to |
| this report was required." |

| determined that an update to this event report was required." |

| the system inoperable at 1420 [CDT] on 8/28/99. LCO required fire patrols |
| of the affected area were initiated. The PSS determined that an update to |
| this report was required." |

| "Two adjacent sprinkler heads on C-337 System D-15 were identified to have |
| corrosion and the PSS declared the system inoperable at 0925 [CDT] on |
| 8/29/99. Also two adjacent heads on C-337 System D-10 were identified to |
| have corrosion and the PSS declared the system inoperable at 1450. LCO |
| required fire patrols for both of the affected areas were initiated in the |
| time frame required by TSR. The PSS determined that an update to this |
| report was required." |

| FAILURE OF 13 HIGH PRESSURE FIRE WATER SPRINKLER SYSTEMS TO MEET SYSTEM |
| OPERABILITY REQUIREMENTS (24-Hour Report) |
| |
| The following text is a portion of a facsimile received from Portsmouth: |
| |
| "On 08/10/99 at approximately 1600 hours, the Plant Shift Superintendent |
| (PSS) was notified that 13 sprinkler systems associated with the High |
| Pressure Fire Water (HPFW) system were not capable of meeting system |
| operability requirements. This condition was discovered as a result of an |
| engineering review of individual sprinkler heads that had previously been |
| identified as exhibiting corrosion around the valve seat. The engineering |
| review was conducted following the discovery that similar corrosion problems |
| experienced at the Paducah Gaseous Diffusion Plant resulted in sprinkler |
| heads being unable to actuate at normal pressure. An initial screening of |
| PORTS sprinkler heads known to exhibit corrosion was conducted, and systems |
| where the potential for two or more adjacent heads to be corroded were |
| inspected. This identified 11 systems in the X-333 and 2 systems in the |
| X-326 that did not meet the sprinkler system operability requirement that no |
| two adjacent sprinkler heads be inoperable." |

| "The PSS declared the affected sprinkler systems inoperable, and TSR |
| required actions were completed. Further inspections of sprinkler systems |
| are planned. This report will be updated if additional sprinkler systems |
| are determined to be inoperable." |
| |
| "There was no loss of hazardous/radioactive material or |
| radioactive/radiological contamination exposure as a result of this |
| event." |
| |
| Portsmouth personnel notified the NRC resident inspector as well as the |
| onsite Department of Energy representative. |

| |
| During continuing walkdowns, HPFW system #398 was discovered with two or |
| more (3) adjacent sprinkler heads with mineral deposits. This HPFW |
| sprinkler was declared inoperable at 1038 hours and was repaired at 1407 |
| hours. |

| "During continuing walk downs of the HPFW system, systems 194, 196, 197, |

| system operability. Systems were made inoperable at 1452 on 8/27/99. All |
| sprinklers were repaired and system operability made at 0550 8/26/99." |

| UNUSUAL EVENT DUE TO LOSS OF MOST CONTROL ROOM ANNUNCIATORS |
| |
| An Unusual Event was declared at 2155 EDT due to an unplanned loss of |
| greater than 75% of the control room safety system annunciators for longer |
| than 15 minutes. The licensee reported that the annunciators are currently |
| inoperable, and that additional panel walkdowns are being performed. The |
| unit is currently stable in Hot Standby following a reactor trip earlier |
| today (see EN 36104). The NRC resident inspector has been informed, and is |
| in the control room. |

| The plant conditions are improving and the plant is stable in Hot Shutdown. |
| The annunciators have been restored. The previous problem with 480 VAC |
| busses on Event #36104 has been found to be related to the loss of the |
| annunciators. Further evaluation is ongoing and the licensee expects to |
| exit the Unusual Event soon. |

| OFFSITE NOTIFICATION DUE TO SMOLDERING EMBERS |
| |
| At the Big Rock Point Restoration Project, some smoldering embers in a |
| pipeway of the recirculation pump room generated smoke. The Charlevoix |
| Township fire department responded to site, but the on site fire brigade was |
| able to take care of the problem. The problem was due to main steam pipe |
| cutting activities heating the pipe insulation cloth coverings to the point |
| of generating embers. There was no fire and no entry into emergency |
| classification. |
| |
| The licensee notified the NRC Resident Inspector. |

| An inboard primary containment isolation system (PCIS) Group III (reactor |
| building ventilation) isolation occurred due to an inadvertent loss of power |
| to the 'A' reactor protection system (RPS) bus during a plant modification. |
| All affected components responded as designed. The 'A' RPS bus was |
| reenergized, and all components were restored. The NRC resident inspector |
| has been informed of this event by the licensee. |

| MICROFILTRATION SYSTEM pH PROBES DISCOVERED TO BE OUT OF TOLERANCE |
| |
| "On 8/31/99 at 1200 hrs, the Plant Shift Superintendent was notified that a |
| previously discovered X-705 Microfiltration System (MFS) pH probe out of |
| tolerance condition likely existed during a time period when the MFS was in |
| operation. The out of tolerance condition was in a non-conservative |
| direction and previously discovered during a routine weekly surveillance on |
| 8/27/99. An evaluation to determine the condition existed while the MFS was |
| in operation was initiated at that time. Engineering suspected that the |
| probe calibration may have been affected when the probes were exposed to a |
| high pH solution on 8/23/99. Subsequent discussion with the probe |
| manufacturer confirmed that a high pH solution could affect probe |
| calibration. As a result, it was concluded that the out of tolerance |
| condition likely existed from approximately 8/23/99 to 8/27/99. |
| |
| "The out of tolerance condition affected 2 of the 3 pH probes (2 out of 3 |
| logic required to activate the safety system). The as found data for the 2 |
| probes indicated they would actuate at 6.73 pH and 6.65 pH, which is below |
| the 7 pH required setpoint. A review of the MFS operational history |
| indicated that the MFS pH was maintained above a 7 pH during the time the |
| probes are suspected to have been out of tolerance." |

| INADVERTENT ACTUATION OF EMERGENCY SIRENS DURING TESTING |
| |
| "During the routine weekly testing of the emergency response sirens |
| (continuity test of siren circuitry), WNP-2 inadvertently activated the |
| audible offsite sirens. The sirens were activated at 1100 hours PDT, and |
| sounded for three minutes. |
| |
| "WNP-2 notified the following government agencies of the inadvertent |
| activation of the emergency response sirens: |

| "Benton County Southeast Communications (911 Center) |
| "Franklin County Emergency Management |
| "United States Department of Energy (Hanford Operations-Occurrence |
| Notification Center) |
| "Washington State Department of Emergency Management |
| |
| "In addition, a notification was made to the local radio station responsible |
| for emergency-broadcasts, with a media/press release to local newspapers. |
| |
| "All offsite agencies and media were advised that the siren activation was |
| inadvertent and not in response to an actual emergency. In addition, offsite |
| agencies were instructed that no emergency response actions were required." |
| |
| The NRC resident inspector has been informed of this event by the licensee. |

| RADIATION MONITOR CABLES DO NOT MEET 10 CFR 50 APPENDIX R SEPARATION |

| "This notification is for San Onofre Units 2 and 3 and is being made In |
| accordance with 10CFR50.72(b)(1)(ii)(B). |
| |
| "Southern California Edison is in the process of implementing a change to |
| the radiation monitoring system at Units 2 and 3. To complete this change, |
| SCE installed a temporary panel in the control room area and removed the |
| original panels and some cable wraps. On August 31,1999, shortly before |
| midnight, licensee personnel noticed that several cables for Train A, Train |
| B, and Train X (e.g., various low voltage electrical power cables and |
| instrument signal lines) appeared to not meet the separation criteria of |
| Regulatory Guide 1.75, 'Physical Independence of Electrical Systems.' |
| Several of the affected cables provide actuation for Engineered Safety |
| Features (e.g., control room isolation and containment purge isolation). The |
| Updated Final Safety Analysis Report for Units 2 and 3 indicates these |
| systems comply with IEEE-297, 1971, 'Criteria for Protection System for |
| Nuclear Power Generating Stations' which is referenced by Regulatory Guide |
| 1.75. Consequently, at 1526 PDT, SCE concluded this condition was reportable |
| in accordance with 10CFR50.72(b)(1)(ii)(B) as a condition 'outside the |
| design basis of the plant'. |
| |
| "Shortly after identifying this condition, control room operators (1) placed |
| Train A of the Control Room Emergency Air Cleanup System in service, and (2) |
| closed and removed power from the containment minipurge valves. SCE |
| determined that this condition has only existed since August 27, 1999. SCE |
| has also confirmed that the affected components remained capable of |
| performing their intended safety functions and, therefore concludes the |
| safety significance of this occurrence is minimal." |

| The NRC resident inspector has been informed of this notification by the |
| licensee. |
