SUBJECT: STAFF REQUIREMENTS - SECY-06-0055 - REPORT TO CONGRESS ON ABNORMAL OCCURRENCES: FISCAL YEAR 2005

The Commission has approved issuance of the FY 2005 Report to Congress on Abnormal Occurrences subject to the changes noted below.

Changes to the Report to Congress on Abnormal Occurrences

  1. Page iii, paragraph 2, line 4, insert a footnote after “brachytherapy” which reads: Brachytherapy means a method of radiation therapy in which sources are used to deliver a radiation dose at a distance of up to a few centimeters by placement of sources on the body surface, in natural body cavities, or by placement directly in tissues.

  2. Page xi, insert in the proper location, centigray and its abbreviation cGy.

  3. Page 3, paragraph on "Nature and Probable Consequences", revise lines 4 and 5 to read ' ... sources, which migrated had the ability to migrate from the ....'

  4. Page 4, paragraph on "Nature and Probable Consequences", line 6, delete "to".
