Investigator Request Form
This form is used to identify the level of data that each investigator [and
his/her staff] will require for entry into the FACILITY database
Training Request Form
Training is mandatory for all users of the FACILITY database to ensure the
integrity and accuracy of the data maintained in the system. This form is used
to document training for those individuals that will be entering data into the
FACILITY database
User Account Request Form
After completion of your FACILITY training session[s], you are authorized to
request access to the FACILITY database and will be instructed by the designated
trainer to complete this account request form
Microchip Request Form
Investigators proposing to use microchips to identify their animals, must submit a request to add the implant procedure to his/her approved Animal Study Proposal [ASP] in advance. This form is used to initiate the ASP modification request.
Request for Customized USER ID Drop-Down Options
This form is used to request customized USER ID drop-down options [up to four]
for individual animal groups in the FACILITY database
Inventory Log Sheet
This form is used by the technical staff [not actively tracking colony data in
the FACILITY database] to manually log animal data for inventory purposes
Wean-Tail Log Sheet
This form is used by the technical staff [not actively tracking colony data in the FACILITY database] to manually log animal weaning data for inventory purposes.
Transfer Log
This form is used by the technical staff [not actively tracking colony data in the FACILITY database] to manually log animal transfer data within the facility for inventory purposes. [Not to be used for inter-building transfers]
Issue Reporting Form
This form is used to report data and/or program related issues regarding the FACILITY database, Supplemental, or the LASP Online Access System.
