Who Should Attend

The OMH 2009 Summit is open to a wide range of participants from across the health disparities policy, program and research community, including State Offices' of Minority Health program staff, health care professionals, community partners, project staff, civic leaders, advocates and federal staff.

Note: Regular online registration closes on February 18, 2009, after this time you may be able to register onsite (space permitting). If your registration information is received after this date, you will not be included in the participant directory.

Awards Reception Tickets

The Awards Reception is set to occur Thursday Evening, 2/26/09. If you would like to bring a guest(s) to the Awards Reception, you will need to purchase additional tickets. Guest Tickets are $175 each, and can be purchased through the online registration and payment process. A limited number of Guest Tickets will be sold on site, and only if available. In order to guarantee your spot at this exciting event, be sure to purchase all your tickets online!

Continuing Education Credits

This Summit will offer the following CEU's
  • Physicians - This activity has been approved for AMA PRA Category 1 Credit(s)TM.
  • Pharmacists - Continuing Pharmacy Education will be available for this activity.
  • Nurses, Dieticians, Social Workers, and Certified Health Educators - Continuing education will be offered for this activity.

Payment Method

Full Registration and payment are now being accepted. Payment types accepted are American Express, Mastercard, Visa and Check. If you are paying by credit card, please note that our co-sponsor, NCQA, will be listed on your credit card bill for collecting registration fees.

Substitutions and Cancellations

Substitutions can be made at any time before February 18, 2009, by calling our toll-free hot line at 1-877-772-3044, or by sending an email with your name and registration number, and the substitute's registration information to 2009Summit_registration@team-psa.com. Substitutions may not be included in the participant directory, if we do not receive all necessary information by February 18, 2009. After this date they will have to register onsite, and provide payment. If the person they are substituting for has paid online, the substitute must bring a copy of the registration and payment confirmation.

If you are canceling your registration and require a refund, call our toll-free hotline at 1-877-772-3044 or email us at 2009Summit_registration@team-psa.com. Refunds will be given until February 18, 2009. A $30.00 per registration cancellation fee will be assessed. After February 18, 2009 no refunds will be given.
