Notice Under 37 CFR 1.251 - Abandoned Application

Used by the applicant to assist the USPTO in reconstructing a current copy of a missing patent or application file. Used by the USPTO to notify the applicant that the application or patent file is unlocatable and to request a copy of the applicant's or patentee's record of the application or patent file in order to reconstruct the file of an unlocatable application or patent.
