Grantee Performance Report Information

Javits 2004 Cohort

The 2006-2007 Annual Performance Report for the Javits Program is due by March 30, 2007.

Javits 2005 Cohort

The 2006-2007 Annual Performance Report for the Javits Program is due by May 31, 2007.

ED has developed a new Annual Performance Report (ED Form 524B). This report replaces the narrative report that was used in the past. This report is enhanced with fields to show quantitative and qualitative data. Versions of the report are available through the Department's Grant Application and Other Forms website, as well as instructions for completing the form.

The report may be submitted on-line through the department's e-grants system and accessed through e-Reports. On-line submission is optional. If submitting a paper performance report to ED, please provide one original and two copies to the following address:
