Since
1981 the Housing Authority of Portland's (HAP) Congregate Housing
Services Program (CHSP) has served hundreds of frail elderly and
disabled people with critical stabilizing services. These services
allow residents to age-in-place and live independently so that some
program participants are able to completely avoid institutionalization.
Initially
the program started with 30 slots at Northwest Tower, a 13-story
apartment building. The CHSP at Northwest Tower was HUD funded,
and residents paid a sliding scale portion of the fee. In 1993 HAP
secured a second HUD CHSP grant that funded ninety slots at three
additional high-rise buildings: two public housing communities,
Dahlke Manor and Holgate House, and an affordable community, Unthank
Plaza. CHSP now has a total of 120 slots in four buildings and serves
approximately 100 residents annually.
HAP
has been challenged to operate within the constraints of decreasing
federal revenue while continuing to serve residents most in need.
To reduce costs and to leverage comprehensive support for participants
while maintaining quality core services, HAP has contracted with
a local service provider, Portland Impact. Portland Impact provides
a range of services and implements the CHSP program.
Services
provided include:
oSpecialized food services
oHealth and wellness services
oSocialization events, i.e., trips to the zoo and coast
oAccess to transportation
oAccess to benefits and law specialists
The
goal of CHSP is to improve the participant's quality of life by
delivering services in a compassionate, efficient and cost effective
manner while promoting independence and personal dignity. HAP is
very pleased to be able to offer the CHSP program to the Portland
community.
