For
28 years the Congregate Housing Services Program (CHSP) at Hale
Mahaolu in Maui, Hawaii has provided meals, case management, and
personal care services to eligible residents at our Akahi and Elua
housing sites.
Each
year through our CHSP we serve an average of 50 frail elderly and
disabled individuals with comprehensive services to meet their health,
safety, and/or care needs. CHSP provides professional case management
through the volunteer Professional Assessment Committee (PAC) to
identify those needing assistance and recommends appropriate levels
of services. CHSP collaborates with local service providers to make
arrangements for our residents' housekeeping, transportation, legal,
and other needs. The CHSP is a cost effective alternative to institutionalization.
A
major component of the CHSP is our meals program where staff is
on duty seven days a week to prepare and deliver meals with nutritional
low fat, low salt, and low sugar food items. Our CHSP provides meal
service delivery directly to our vulnerable CHSP residents who live
in apartment units situated throughout the housing sites. It is
difficult for physically impaired tenants to walk the distance to
the congregate meal site located in a separate building. Inclement
weather makes it even more difficult and dangerous for residents
to leave their apartments.
We
provide as much individualized service as possible to enhance the
nutrition and health of our meal participants. Residents enjoy hot
meals from the menu of the day or may choose to order a substitute
meal. Diabetic meals are available and consideration is given to
other special medical diets, ethnic food preferences, religious
restrictions, and individual preferences.
With
affordable in home services provided through CHSP our frail elderly
and disabled individuals are able to live independently and enjoy
peace and privacy surrounded by their own personal possessions.
