AERIFORM CUSTOMERS IN HOUSTON, PORT ARTHUR AND BEAUMONT ALERTED TO HAZARDOUS AIR CYLINDERS

On March 21, 2000 a double fatality at Equistar in Channelview, Texas, occurred. Two painters, employed by AIS/Basic, were wearing sandblast hoods connected to compressed air cylinders. Preliminary tests show a low level of oxygen in the cylinders. The cylinders were filled by Aeriform.

Aeriform in Houston, Texas, has shipped 24 cylinders of class D breathing air to about 30 companies in the Houston, Port Arthur and Beaumont areas.

The air cylinders may not contain a sufficient amount of oxygen and could be hazardous.

The cylinders were part of lot number C860-2-00-39RS, which consisted of three clusters of 12 cylinders that were filled at Aeriform on Feb. 8, 2000. Stenciled at the bottom frame of the clustered cylinders are either the numbers 9063 or 9065.

The Occupational Safety and Health Administration and Aeriform request that companies take the cylinders from service and return them to Aeriform immediately for testing. Contact Aeriform immediately at (713) 926-3166.
