  • AK, CO, DE, HI, IA, IL, KS, LA, ME, MI, MN, MT, NH, NV, OH, OK, OR, WV, and WY did not respond, or provided only a limited response, to the reporting population questions in 1990 through 1994
  • DC did not respond or provided only a limited response to these questions in 1990 through 1993
  • AZ, CA, CT, FL, KY, PA, PR, SD, TX, UT, and WA did not respond, or provided only a limited response, to the reporting population questions in 1991 through 1994
  • NJ, NM, and RI did not respond, or provided only a limited response, to the reporting population questions in 1991 through 1993
