Project Description: The purpose of this research is to develop an evidence-based childbirth decision support tool to weigh the consequences for various childbirth methods, and test its effectiveness to improve patients' shared-decisionmaking and satisfaction, and reduce feelings of failure, anxiety and postpartum depression. This Web-based tool will be widely accessible, providing an opportunity to overcome the economic, racial, linguistic, and ethnic disparities that currently exist in prenatal education and health outcomes.

Career Goals: Dr. Guise is Associate Professor in the Department of Obstetrics and Gynecology (OB/GYN) at Oregon Health and Science University. She plans to become an independent physician-scientist researcher and mentor in academic OB/GYN. In addition, she would like to become an expert in translating research findings into practical tools for use by practitioners, patients, and policymakers; and plans on formal coursework in decision sciences and medical informatics to augment her skills in clinical epidemiology, medical ethics, biostatistics, and secondary analysis. Her long-term goal is to set a foundation for academic leadership in the integration of health services research into clinical and academic OB/GYN.

Progress to Date: Dr. Guise completed the Agency for Healthcare Research and Quality (AHRQ) funded evidence report of Vaginal Birth after Cesarean, which has resulted in the submission of seven manuscripts. She also developed a survey to assess patient satisfaction with the information they receive and priorities in decisionmaking for all women with prior cesarean delivery. Dr. Guise is mentoring others.

Future Plans: Dr. Guise will continue her work in bringing the best evidence to patients and providers for clinical decisionmaking. She is also continuing to pursue epidemiological, informatics, and clinical research to improve the health and safety of women and children in childbirth.
