Vermont
Background: Since 2000, the Vermont Department of Health has
been funded as a standard project.
Lifestyle Intervention: Women receive the self-help Healthy
Choices From Ladies First notebook, a letter detailing their
screening numbers that they can compare to recommended levels in their
notebook, a pedometer with instructions and a magnetized wipe-off board
to record daily diet and physical activity. The package is mailed to a
women after she is screened by her provider. Follow-up phone calls,
utilizing motivational interviewing, are conducted by Vermont Health
Department nutritionists.
Screening: Cardiovascular risk factor screening includes blood
pressure, glucose, cholesterol and other lipid testing as well as weight
assessment (BMI), personal and family medical history, tobacco use,
diet, and physical activity.
Sites: Vermont contracts with over 540 providers that offer
WISEWOMAN screening and treatment or referral services.
Key Partners: American Heart Association, Community Health Center -
Burlington, Northwestern Medical Center, Vermont Coalition of Clinics
for the Uninsured (VCCU).
