My benefits began before July 2, 1986. My benefits began between July 2, 1986 and December 31, 1986. My benefits began January 1, 1987 or later.

I retired for disability. I retired for reasons other than disability.
My spouse died before retiring. My spouse retired for reasons other than disability. My spouse retired for disability and was not later eligible for a regular retirement. My spouse retired on disability and was later eligible for a regular retirement.
