Molecular surveillance within year 2002 and 2003 invasive pneumococcal isolates for capsular switching events from 7-valent conjugate vaccine serotypes to those not covered by this vaccine.

Abstract: Exacting genotypic surveillance is necessary to monitor effects of selective pressures from a new vaccine (Prevnar) on pneumococcal population genetics. Genotypic surveillance allows detection of horizontal transfer events that result in changes of serotype within virulent clones, and detection of new antibiotic resistant strains. We have determined the pre-Prevnar population genetic structure of invasive pneumococci in the United States (in press). We are conducting population based genotypic analysis of Active Bacterial Core Surveillance (ABCs) year 2001 invasive isolates from the pediatric population and selected adults (approximately 1100 isolates), as well as years 2002 and 2003 invasive isolates from the pediatric population (and selected adults) that are of serotypes not included in Prevnar. Using this data, we will assess selective effects of Prevnar on the pneumococcal population genetic structure through monitoring fluctuations within existing clonal populations and detecting newly emerging clones. We have established the only population-based invasive surveillance in the United States that employs multilocus sequence typing, which allows conveying pneumococcal strain distribution information in a non-subjective and easily understood format.
