Use of Current PHS 416-1 and PHS 416-9 Individual National Research Service Award (NRSA) Applications and Related Forms (OMB NO. 0925-0002)

The Application for a Ruth L. Kirschstein National Research Service Award Individual Fellowship (PHS 416-1) and Progress Report for Continuation Support (416-9) currently in use were revised June 2002 and approved for use through June 30, 2005. The forms have again been revised and approval is pending at OMB.

In the interim, until approval is granted from the OMB and until further notice, the current versions of the PHS 416-1, 416-9 should be used. This extension also applies to the following related forms: PHS 416-5—Activation Notice; PHS 416-7--Termination Notice; PHS 6031—Payback Agreement; and PHS6031-1—Annual Payback Activities Certification.

Once OMB approval is received, the new forms will become available for use and will be accessible on the NIH web site. There will be a transition period when both the new or old forms will be accepted. Further guidance will be provided at that time.
