PREP Supplemental Instructions for Form PHS 2590


(For Noncompeting Grant Progress Reports)

  1. Complete the Face Page (Form Page 1) except for items 8a. and 8b.

  2. Complete the Budget Pages (Forms 2 and 3) only if any of your responses to the Streamlined Noncompeting Award Process (SNAP) are "yes."

  3. Provide Biographical Sketches (Form Page 4) for key personnel who are newly added in the current fiscal year.

  4. Complete the Progress Report (Form Page 5), and answer the three SNAP questions (see PREP Sample Table 1).

    The Progress Report should be no more than eight pages (including graphs and/or tables). If needed, use the continuation pages available at http://grants.nih.gov/grants/funding/2590/continuation.doc. Number the pages sequentially (1, 2, 3, etc., do not use 2a, 2b, 2c, etc.).

    • Provide a brief overall summary of the progress made during the current reporting period only. Describe the major student development activities that contributed in achieving the goals set by the program. Briefly describe any challenges faced by students when applying to Ph.D. and/or predoctoral training programs. Include the current status of previous PREP participants.

    • Restate the goals, specific measurable objectives, anticipated milestones, and outcomes proposed in the application. Provide this information as outlined in PREP Sample Table 2.

    • Briefly describe other institutional student development and training programs that are synergistic with PREP. Describe the PREP students' participation in these activities as well as their interactions with predoctoral students in the participating departments.

    • Describe any activities within the institution geared toward increasing faculty members' awareness of diversity issues as well as increasing the number of underrepresented students within the participating departments.

    For the evaluation of the program, provide the following information:

    • Summary of the evaluator’s findings.
    • Program actions in response to these findings.
    • Institutional actions to address identified issues.
  5. Complete the Checklist Page (Form Page 6) only if there is a change in performance site(s) that will affect the facilities and administrative costs and/or if program income is anticipated. If program income is anticipated, the Checklist should reflect the amount and source(s).

  6. Complete the Key Personnel Report (Form Page 7). List all key personnel (including PREP students) for the current budget period.
