How to Request an NCD

Requesting a National Coverage Determination (NCD) is a formal process outlined in the September 26, 2003 Federal Register Notice and in several subsequent guidance documents. We strongly encourage those wishing to request an NCD (e.g. industry, beneficiaries, or others who may be unfamiliar with the process) to contact us informally prior to the formal request to learn more about the process. Informal and formal requests may be submitted through our website or emailed to cms_caginquiries@cms.hhs.gov.

NOTE: We are requesting that all documents sent to CMS as part of the NCD request be 508 compliant.

Section 508 is an amendment to the Rehabilitation Act of 1973 which requires that Federal websites be accessible to people with disabilities. Web accessibility means that people with disabilities can perceive, understand, navigate, and interact with the Web. In accordance with HHS guidelines, CMS will only post materials that are 508 compliant.
