Summary
The CSB investigated a combustible dust explosion and fire at CTA Acoustics, a fiberglass insulation manufacturing plant in Corbin, KY. The investigation found that CTA management programs and maintenance procedures were inadequate. The manufacturer's product stewardship program lacked explicit communication on the explosive hazards of phenolic resin. Furthermore, building design and modifications did not effectively address the hazards associated with combustible dusts. The CSB issued safety recommendations based on these root causes.
