The CSB investigated a fire at the Bethlehem Steel Corporation's Burns Harbor mill in Chesterton, IN. The investigation found that management systems for the supervision, planning, and execution of maintenance work, as well as systems for monitoring and controlling hazards were inadequate. In addition, the facility did not have a program to identify and address hazards that might be created by decommissioning and demolition operations. The CSB issued safety recommendations based on these root and contributing causes.
