Summary
The CSB investigated a high explosives accident at Sierra Chemical Co. in Mustang, NV. Several root and contributing causes of the incident were discovered. They include: inadequate process hazard analyses, training programs, and written operating procedures. In addition, the facility was built with an inadequate siting plan, no systematic safety inspection or auditing program existed, and regulatory oversight was insufficient.
