Summary
The CSB investigated an explosion and fire at Morton International, a dye factory in Paterson, NJ. Hazard analyses that failed to recognize the reactive hazards of the Yellow 96 dye production process and process safety information that did not warn personnel of the potential for a dangerous runaway reaction were the root causes pf the incident. The fact that previous operational deviations were not analyzed for hazards, and Morton did not follow management of change procedures also contributed to the incident. The CSB issued safety recommendations based on these root and contributing causes.
