CDC's Steps Program operates a cooperative agreement that provides funding to communities nationwide to support evidence-based community interventions. These interventions focus on reducing the burden of obesity, diabetes, and asthma, as well as addressing three related risk factors: physical inactivity, poor nutrition, and tobacco use.

Actions Guides to Improve Community Health

Handbook Cover CDC's Steps Program and Partnership for Prevention have developed implementation guidance ("Action Guides") on five effective community-level health promotion strategies relating to diabetes self-management, physical activity, and tobacco-use treatment.
