Target Population
The programs proposed are specifically tailored to inner city, low-income populations with a large portion of non-English speaking individuals. This targeting of multi-cultural communities is in concert with the goals of Healthy People 2010.
Proposed Strategy
This proposal utilizes a comprehensive, multi-disciplinary approach to care in the clinic system of a major medical center in the targeted areas, along with culturally sensitive public awareness and community outreach campaigns. There is an emphasis on language-specific community outreach and public awareness campaigns being proposed to reduce asthma morbidity and mortality in the target communities.
The strategy will involve an intervention project tied into two common Advisory Boards and two major community partners that umbrella over 75 smaller community groups.
The proposed strategy will focus on three NAEPP goals:
- #1 -- To promote asthma awareness
- #2 -- To diagnose and manage asthma; and
- #3 -- To prevent recurrent exacerbation of asthma.
Anticipated Outcome
The aim of the program is to enroll high-risk patients into a culturally appropriate, multi-faceted program for asthma management. The program incorporates appropriate medical care, asthma education, psychosocial screening, environmental assessment, and referrals to appropriate social service agencies. The success of the intervention will be measured by frequency of asthma exacerbations, frequency of emergency department visits and hospitalizations, physician management of asthma patients, and patient compliance with treatment plans.
